Need to Make a Referral?
If you are a member of the counseling team and would like a student to receive therapeutic services from a Daybreak Health clinician, please click the link below to submit a referral. (Before submitting the referral, please speak with the student’s parent or guardian about the therapeutic services available through the Mobile Wellness Center and confirm that they are interested in having their student receive these services.)
